Sarcopenia: Causes of Muscle Loss and Solutions

Sarcopenia is the age-related loss of skeletal muscle mass, strength, and physical performance. It becomes more common with aging, but it can also occur earlier because of inactivity, illness, poor nutrition, or hormonal changes.

Common causes

  • Aging: Muscle protein synthesis and the body’s ability to repair muscle gradually decline.
  • Physical inactivity: Sedentary behavior and prolonged bed rest can cause rapid muscle loss.
  • Insufficient protein or calories: Not eating enough protein, overall food, or essential nutrients makes it harder to maintain muscle.
  • Chronic diseases: Conditions affecting the heart, lungs, kidneys, liver, or metabolism can contribute.
  • Hormonal changes: Lower levels of anabolic hormones such as testosterone, estrogen, and growth-related hormones may play a role.
  • Inflammation: Chronic inflammation can increase muscle breakdown.
  • Vitamin D deficiency: Low vitamin D may be associated with reduced muscle function, particularly in older adults.
  • Smoking and excessive alcohol: Both can negatively affect muscle health.

What helps prevent or reverse muscle loss?

1. Resistance exercise — the most important intervention

Strength training 2–3 times per week can stimulate muscle growth and improve strength. Examples include:

  • Squats or sit-to-stand exercises
  • Resistance-band exercises
  • Rows
  • Push-ups or chest presses
  • Hip-hinge/deadlift movements
  • Calf raises

Progressively increasing resistance is generally more effective than simply doing lots of repetitions with very light resistance.

2. Eat enough protein

A useful target for many older adults is roughly 1.0–1.2 g of protein/kg body weight/day, with higher amounts sometimes appropriate during illness or when trying to rebuild muscle. Spread protein across meals rather than consuming almost all of it at one meal.

Good sources include eggs, milk/yogurt, fish, chicken, lean meat, beans, lentils, soy, and other legumes.

3. Avoid unnecessary weight loss

If you’re losing weight unintentionally, particularly if you’re also becoming weaker, it’s worth discussing this with a healthcare professional. Calorie restriction can accelerate muscle loss unless nutrition and resistance training are carefully managed.

4. Maintain adequate vitamin D and other nutrients

A balanced diet is preferable to taking multiple supplements routinely. Vitamin D supplementation may be appropriate if you are deficient, but the dose should be based on your circumstances and, when appropriate, blood testing.

5. Stay physically active

Walking, climbing stairs, balance exercises, and regular daily movement complement strength training and help preserve independence.

6. Address underlying medical problems

If muscle loss is substantial or occurring unusually quickly, a clinician should look for potentially treatable causes such as malnutrition, thyroid disease, medication effects, or chronic illness.

When to get checked

Consider medical evaluation if you notice progressive weakness, difficulty getting up from a chair, climbing stairs, carrying groceries, or repeated falls, especially if accompanied by unexplained weight loss.

A clinician can assess muscle strength and physical performance and may use tests such as handgrip strength, chair-stand testing, and measurements of muscle mass to determine whether sarcopenia is present.

Bottom line: For most people, the combination of progressive resistance training + adequate protein and calories + regular physical activity + treatment of underlying health problems is the foundation for maintaining or rebuilding muscle.
